The Dyslexia Assessment evaluates reading and related academic skills to identify patterns consistent with dyslexia and guide educational planning. It is not a neuropsychological evaluation and does not include medical diagnosis. Testing typically takes 2–4 hours across one or two sessions and may include the Tests of Dyslexia (TOD) along with additional reading and phonological processing measures. A results meeting is held 1–3 weeks after testing. Parents/guardians agree to ensure the student is prepared, share relevant information, and remain available during the appointment. A non‑refundable $250 deposit is required to reserve testing, with the remaining $800 due at the results meeting. Cancellations with less than 48 hours’ notice may require an additional deposit. Results reflect performance during testing and may not determine school eligibility. All information remains confidential, and signing indicates consent to the assessment and acknowledgment of the scope, timeline, and payment terms.

1) Purpose and Scope of Services
Provider will conduct a Dyslexia Assessment to evaluate reading and related academic skills and to identify patterns consistent with dyslexia and associated learning needs. Results are intended to support educational planning and intervention recommendations.
2) Important Clarification: Not a Neuropsychological Evaluation
Parent/Guardian/Client understands and agrees that this service is a Dyslexia Assessment, not a Neuropsychological Evaluation.
A neuropsychological evaluation typically assesses broader areas such as cognition, memory, executive functioning, attention, and social/emotional functioning and may include components beyond the scope of this assessment. This Dyslexia Assessment focuses on dyslexia-related skill areas.
Provider does not provide medical services and does not diagnose medical conditions.
3) Tests and Procedures Included
The assessment includes administration, scoring, and interpretation of:
- Tests of Dyslexia (TOD)
Possible additional assessments could include:
- Gray Oral Reading Test (GORT)
- Comprehensive Test of Phonological Processing (C-TOPP)
- Word Identification and Spelling Test (WIST)
Provider may also include brief informal measures (e.g., interview, record review, observation) as appropriate to support interpretation of results.
4) Time Commitment and Session Structure
Parent/Guardian/Client understands that:
- Testing will take approximately 2-4 total hours.
- Testing may be completed in one or two sessions, depending on student stamina, scheduling, and Provider recommendation.
- Breaks will be provided as needed.
5) Timeline and Results Review Meeting
Testing Sessions: Scheduled for
[Date(s)/Time(s)] (appointments are reserved after the deposit is received).
Results Review Meeting:
A one-hour meeting will be scheduled approximately 1–3 weeks after testing to review results, answer questions, and discuss recommendations and next steps.
The timeline may be impacted by illness, missed/rescheduled sessions, delays in receiving requested records, or other circumstances outside Provider’s control.
6) Parent/Guardian/Client Responsibilities
Parent/Guardian/Client agrees to:
- Ensure the student arrives on time, rested, and fed.
- Inform Provider of factors that may affect testing (e.g., illness, medication changes, vision/hearing concerns, significant stressors).
- Remain available during the testing appointment. You may remain in the office or be accessible by phone
7) Fees, Deposit, and Payment Terms
Non-Refundable Deposit (required to reserve appointments): $250
- The $250 deposit is required to book and hold assessment time.
- The deposit is non-refundable, including if Parent/Guardian/Client cancels or reschedules.
Remaining Balance (due at Results Review Meeting): $800
- The remaining $800 is due at the one-hour Results Review Meeting (when results are discussed).
- Provider will not release results/recommendations and/or any written summary/report (if applicable) until payment is received in full.
Payment Methods Accepted: After dates and times are agreed upon Parent/Guardian/Client will pay $250 via the LTR website using Venmo, PayPal or a credit card, to reserve the testing time. There is a 2.9% credit card processing fee.
8) Rescheduling, Cancellation, and No-Show Policy
Because assessment appointments reserve a significant block of time:
- Parent/Guardian/Client must provide at least 48 hours’ notice to reschedule a testing session or results meeting.
- Changes made with less than 48 hour notice may require forfeiture of the reserved time and/or may require an additional deposit to rebook, at Provider’s discretion.
- No-shows may be treated as same-day cancellations and may require an additional deposit to reschedule.
If Provider must reschedule due to illness or emergency, Parent/Guardian/Client will be offered the next available appointment.
9) Nature and Limitations of Results
Parent/Guardian understands:
- Test results reflect the student’s performance on the day(s) of testing and may be influenced by fatigue, attention, motivation, anxiety, health, or other factors.
- Schools and agencies have their own criteria for services or accommodations; Provider cannot guarantee eligibility determinations.
10) Confidentiality and Release of Information
Assessment information is confidential and will not be released to third parties (including schools, physicians, tutors, or other professionals) without written permission from Parent/Guardian, except as required by law.
11) Consent for Assessment
By signing below, Parent/Guardian/Client consents to the Dyslexia Assessment described in this Agreement and acknowledges understanding of the scope, timeline, and payment terms.
